Cyber Insurance Requirements: Why Your Renewal Might Get Denied Without MFA

Cyber insurers are scrutinizing security controls far more closely than they did even a few years ago, and the list of cyber insurance requirements now includes specific technical standards that many businesses have not yet fully adopted. Multi-factor authentication has moved from a recommended practice to a near-universal requirement at renewal, and insurers are using its presence or absence to determine whether to issue, renew, or restrict coverage.

Getting ahead of that evaluation requires knowing what today’s insurers are looking for before you sit down to renew.

How Cyber Insurance Requirements Have Changed

Cyber insurance was a relatively straightforward product to obtain not long ago. Underwriters asked general questions, premiums were modest, and most businesses with reasonable revenue qualified without significant difficulty. The surge in ransomware incidents and large-scale data breaches changed that quickly, and the result has been a meaningful tightening of what is required to qualify for coverage and what conditions must be maintained to keep it.

Cyber insurance requirements routinely include documentation of specific security controls, and the renewal process involves detailed technical questionnaires that go well beyond what most businesses expect the first time they work through one. Insurers have become more deliberate about the controls they evaluate because they have more data than ever about which gaps lead to covered incidents. MFA sits near the top of that list for most carriers, and businesses that cannot demonstrate it across the right access points are increasingly running into problems at renewal.

What MFA Is and Why Insurers Evaluate It

Multi-factor authentication adds a second verification step beyond a password before access is granted to an account or system. Rather than relying on something a user knows, it adds something a user has: typically a temporary code sent to a phone or generated by an authentication app, or a hardware key. From an insurer’s perspective, understanding what MFA does for network security is straightforward: it reduces the impact of credential theft, which is the most common entry point for both ransomware and data breach incidents.

A stolen password alone is no longer enough to gain access to a protected account when MFA is in place, and that one control eliminates a significant share of the attack paths insurers are most likely to pay claims on.

Where MFA Requirements Apply

MFA requirements are not satisfied by enabling the feature in a single application. Insurers typically evaluate MFA coverage across a specific set of access points:

  • Email platforms, including Microsoft 365 and Google Workspace
  • Remote access tools and VPN connections
  • Cloud-hosted applications and business management software
  • Administrative and privileged accounts with elevated access rights
  • Remote desktop protocol (RDP) connections

A business that has MFA on email but not on its VPN or administrative accounts may still face questions at renewal. Carriers evaluate these specific access points because they represent the paths most frequently exploited in covered incidents, and a gap in any one of them is enough to raise underwriting concerns.

Why This Evaluation Has Become More Rigorous

The shift in how insurers approach cyber insurance requirements did not happen arbitrarily. Carriers have built out substantial claims data over the past several years, and that data consistently points to the same entry points and the same missing controls. The result is a more systematic underwriting process that treats security posture the same way a commercial property insurer treats fire suppression systems: as a condition of coverage, not a preference.

Businesses that managed their cyber coverage with loose attestations a few years ago are now finding that the renewal application asks pointed, specific questions about which systems have MFA enabled, how backups are stored, and whether endpoint protection is active across all devices. That level of scrutiny is now the baseline, not the exception.

What Happens at Renewal When MFA Is Missing

When an insurer reviews your renewal application and finds gaps in MFA coverage, the outcome is rarely just a warning. Carriers have several responses available when a business does not meet the security baseline they expect.

  • They can decline the renewal outright, which leaves the business searching for coverage under time pressure.
  • They can issue a policy with exclusions that remove certain types of incidents from coverage entirely, with ransomware being a common carve-out for businesses with weak access controls.
  • They can increase the cyber insurance cost substantially to offset the elevated risk a business with documented control gaps represents.

In some cases, a claim filed by a business that did not fully disclose its security posture at renewal can face additional scrutiny at the time of payout. What a cyber insurance policy actually covers is worth reviewing carefully, because the gap between what a policy appears to offer and what it will pay out when controls are missing can be significant. Understanding that distinction before a gap creates a problem is considerably more useful than understanding it after.

Reviewing Your Controls Before the Next Renewal

The most practical approach to cyber insurance renewal is to treat the application as an audit rather than a formality. A pre-renewal review of your own environment gives you time to close gaps before they become underwriting problems, and it positions you to answer the technical questions on a cyber insurance checklist accurately rather than estimating. Carriers can tell the difference between an organization that has reviewed and confirmed its controls and one that is answering the application without full visibility into its own environment.

The following areas are most commonly evaluated in current underwriting reviews:

  • Multi-factor authentication across email, remote access, cloud applications, and administrative accounts
  • Endpoint detection and response on all company devices
  • Regular, tested data backups stored separately from the primary network
  • Employee security awareness training with documented completion records
  • A formal incident response plan that has been reviewed and updated recently
  • Privileged access management for accounts with elevated permissions
  • Email filtering and anti-phishing controls in place

Any gap identified during this review represents the same gap a carrier will find during underwriting. The Nology team provides compliance consulting services that help businesses work through exactly this kind of pre-renewal assessment, identify what needs to be addressed, and document the controls that are already in place.

Make Sure Your Renewal Has No Surprises

Cyber insurance requirements have moved well past the point where general good intentions about cybersecurity satisfy an underwriter. Specific controls are expected, MFA sits near the top of that list, and businesses that arrive at renewal without it face real consequences: denials, exclusions, or a significantly higher cyber insurance cost that reflects the gaps in their security posture. The Nology team works with businesses to assess where their current environment stands against the controls cyber insurers now expect, identify what needs to be addressed before renewal, and build a security foundation that supports better coverage outcomes year-round.

Getting that work done well before your renewal date gives you time to close gaps on your own schedule rather than rushing to respond to an underwriter’s concerns.
